Loss Of Life - USS Galileo

It is with great regret that I inform you of the loss of Lieutenant Commander Evelyn Coleman, assigned as Second Officer to the USS Galileo.

As acting Captain in the absence of Lirha Saalm, currently assigned elsewhere, I have ordered a full investigation into the cause of death, which will be made available to Starfleet Medical and Starfleet Security upon completion.

Initial analysis indicates that the death of Lieutenant Commander Coleman was not accidental, and it is extremely likely that she was killed whilst off-ship. Her time away from the Galileo was authorised by myself, and any repercussions for failing to follow away team protocol should be directed at me.

Our investigations continue.
